  • The discount program is offered for households, which includes EVERYONE in the household even if they are not related.
  • This program is not insurance. It is a discounted savings program, therefore all applicants are guaranteed to be accepted. All ongoing dental problems are ok, except for orthodontic treatment that is already in progress.
  • The program requires that you must use one of the many participating providers. If your provider is not listed, a form can be provided to have the company contact your provider.
  • If you are not satisfied within the first 30 days of becoming a member, you may cancel your membership in writing by mail to 5700 Democracy Drive, Plano, Texas 75024, fax to (469) 229-4595 or email to stop@stopmembership.com. Your membership fee will be refunded. Please be aware that the one-time administrative fee and money paid for healthcare services and products are not refundable. After 30 days membership fees are not-refundable. You may cancel your membership at any time upon written request to AmeriPlan®.

 

Dental Care
Save up to 80% on Cleanings, X-rays, and preventative, maintenance dental work. Also, save up to 65% on all restorative and cosmetic work. Instant savings, all ongoing dental problems are accepted, except for orthodontic treatment already in progress. You may change your dentist at any time.		Prescriptions
Prescription discount savings offers up to 25% off most brand name prescriptions and up to 50% off most generic and many retail pharmacies. The Prescription Program included with the dental program at no additional cost. 
Optical/Vision Savings
Save from 20 to 60% off on contact lenses and glasses. Save up to 30% on eye exams and surgeries. Optical savings are included with the dental program at no additional cost		Chiropractic Care
Free initial consultation, 50% off all diagnostic services and x-rays, with a 30% discount for all other treatment. Chiropractic savings are included with the dental program at no additional cost

The AmeriPlan Health™ discount card programs offer a reduction in the cost of medical, dental, vision,
pharmaceutical and chiropractic services. AmeriPlan Health™ benefit programs are not insurance, nor
is it an alternative to or substitution for insurance coverage. Since it is not health insurance, members
must use an AmeriPlan® provider and they are responsible for payment to the AmeriPlan® providers
at point of service of discount health care fees incurred. If you currently have insurance, you should
be aware that canceling that policy often times makes it difficult to later obtain coverage. AmeriPlan
Health™ recommends that you speak to your insurance agent or your company's insurance
administrator before canceling any health insurance policy for any reason.
